Dushnilin 92983cae83 fix(generator): use predefined DNS rule action for dns_hosts instead of fakeip ...

- Remove broken per-entry fakeip DNS server approach (sing-box allows only one fakeip server)
- Remove dns.hosts map approach (not supported in sing-box 1.13)
- Use action:predefined in DNS rules with zone-file format RR strings
  e.g. 'domain. 60 IN A 1.2.3.4' — tested on sing-box 1.13.14-extended
